Report of Marriage information

Hi!

I wasn't sure which topic to post this in but feel free to move forums if needed but I figured a few people may need some information while processing their adjustment of status for other documents.

 

RE: I want to report my marriage for my husband (Filipino citizen) to the philippine consulate in Los Angeles (where we got married). I currently live in SF

 

DETAILS OF ASCENDENTS

No. 14 -  Do I put our parents' names before they were married or after marriage?

No. 16 & 18 - Name of witness. We had a big ceremony, do I put the officiant as my witness? I didn't have anyone to sign as a witness but we had a big ceremony with lots of witnesses but no written document as to who it actually was.

No. 17 & 19  - I'm assuming current residence.

No. 14: I put my mom's and my wife's maiden name. 

No. 16/18: I just named people that were there at the time. No one actually had to sign it so I just named people that were at out civil marriage. So just pick two friends of y'all to put on there. 

No. 17/19 Residency like SF, CA. I did not put a actual address but a city and state. 

 

 

Do you know if you are going to go in person or mail it in? Did you get married in the Los Angeles Consulate area or the SF area?: Because you can ONLY do the ROM at the consulate that controls the area of your wedding. For Example: My asawa and I got married in NC and we then moved to Los Angeles. So we had to do the ROM at the Philippine Embassy in Washington DC because it's area includes NC.

I will mail it in since I live in SF.

We got married in the LA county and our notary public submitted our license to Los Angeles County Clerk office.

Question: Did the US citizen had to submit an original birth certificate too?

 

Whoever wrote the information for their report of marriage requirements needs to work on their explanations haha. It even mentions birth certificate of husband and wife, then it says it again filipino birth certificate of filipino citizen. lol

No, I just sent in papers with my DL on it and my passport page on it that was it. They just want the Filipino citizen birth certificate. I agree it is not written well at all and what sucks is that the requirements change per different consulates.

Cool, I'll just include my DL too. So they dont need to ask for anything else haha! 

So for our parents names I just put their maiden names. and witness address as City, State of their residency. Thank you so much! how long did your overall process take for this?

We had to mail it in as well and it took a little over a month. There was a issue because my wife's bc original was really in bad shape. So they called us to get a better copy of it. 

 

Make sure to send a self addressed envelope(postage already paid) so they can mail it back to you.

Noted! Thanks! So did you have to notarize all 4 of the Report of Marriage forms? Or just 1 and the rest are still original.

I notarized all 4 of them honestly. Because when dealing with the Philippines Consulate it's better be safe and have it than need it and not have it.

They are talking ROM after getting married.   The form has to be notarized IF not presented in person (and signed there)
