USCIS customer service center tells me that "During AOS application process, you do not need to submit either medical examination report or vaccination supplement if you enter the US with K-3 visa."

I am assuming that it is true because K-3 visa holder submits the sealed medical exam report including vaccination supplement to his/her consulate during the visa processing.

So, the guides and discussions posted here a little bit confusing. I appreciate, if anyone can share his/her experience with me on this issue.

My experience was that my wife sumbitted the DS-3025 vaccination report from her visa medical exam with her I-485. The report was complete, by which I mean that all vaccinations were marked either as having been given or as medically inappropriate and so should be waived, and nothing more was asked of her.

Other people have not been so lucky. The people who open the submission packages and assemble the case file go by a checklist, and they are expecting to see some type of vaccination report from K-visa applicants. Either the DS-3025 that the DoS uses or the I-693A that the USCIS uses. Some of them want to see the I-693A and will send an RFE for it, ignoring the DS-3025.

The same thing can happen again when a case officer is processing the I-485. Even though the original DS-3025 is in the case file, the anecdotal evidence is that sometimes they want an I-693A anyway. They will for sure want an I-693A if the DS-3025 is not complete - shows that some vaccinations are missing.

As you can see, it is a confusing situation, and the more experiences you read about the more different things you will see.

that has been a big concern for me as well. my husband came here on a k3 and they took his sealed envelope at his POE. We have no medical except for the DS3025 ( not sure if that number is correct but it is the vaccination supplement). I consulted with an attorney and was informed that i do NOT submit any medical with AOS but i hear that everybody sends in some type of form from their medical. Since, I do have that copy with my husbands vaccinations, I am going to submit it anyway, regardless what the attorney said- i rather be safe than sorry.

I got a I-693A vaccination supplement done by a U.S. civil surgeon because I didn't want to risk an RFE for that. The surgeon transcribed the information from my vaccination worksheet from my medical abroad.

The instructions for the I-485 state that K1s (and I am taking it to assume K3s as wel) only need to submit a "vaccination supplement". As Yodrak stated, some people are fine submitting the DS-3025 Vaccination Worksheet. But if you want to get technical, the DS-3025 is not a "vaccination supplement" which is what is requested, it's a "vaccination worksheet" even though the information is exactly the same.

I'm surprised at the attorney's advice given that adjustment of status is, by definition, changing from a non-immigrant status to immigrant status. Most non-immigrant categories do not need a physical exam and vaccinations prior to entering the US, which is why I-485 call for a medical report. K-visa people are exempted from the medical report, because they do require an exam to get their visa, but like other non-immigrants they do not require vaccinations to get the visa. The default assumption is that they have not had their vaccinations, and so the USCIS version of the vaccination report is requested as a matter of course.

There are various points of confusion that arise from all of this:

- Does the proof of vaccinantions need to be included with the I-485 application or can it wait to be submitted at the interview?

- If all vaccination have been given prior to entry and are so indicated on the DS-3025, is the DS-3025 sufficient or is an I-693A necessary?

- If all vaccination have been given prior to entry and are so indicated on the DS-3025, why does anything need to be submitted since the original DS-3025 is already in the hands of the USCIS.

It's a very awkward situation. For those who have had all of their vaccinations prudence dictates that they include DS-3025 with their I-485. Additional prudence suggests paying some Civil Surgeon to copy the DS-3025 information onto an I-693A form.

Thank you very much for sharing your knowledge and experience with me. I think, it is a good practice to include the vaccination document with the application form.

I had vaccinations to get K-3 visa at the consulate. I assume that, it is a common procedure followed during the medical exam.

my husband entered on a k3 . I had an attorney do the I130 paperwork which still to this day has never been approved. So i filed a k3 and got my husband here by myself w/o the help of my attorney. so needless to say, because i hired my attorney for I130 and my husband here on k3- i did not retain him for services for k3 ( THAT IS BULL####)!

i asked him about submitting a medical with the AOS and he told me that i dont need to because my husbands medical forms were taken from the POE and will be forwarded to the appropriate place.

This is very confusing for me because i hear from other people's expeirence and I am being told different from an immigration attorney. I do have my husbands DS3025 ( a copy) and i will be submitting that with the AOS regardless of what the attorney said.

Even though his medical records done for his K3 were forwarded, a supplement is still required as outlined in the I-485 instructions. Ignore what the attorney said.

From the I-485 instructions. Substitute "K-1" for "K-3":

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment of status application.
