Good day,

I have already finished filling up my DS-160 form and have already applied for an scheduled appoint at the U.S. Embassy, but I made a mistake on my appointment application for I have entered my middle name instead of my last name but everything in my DS-160 form is correct. What should I do?

You can re-submit a new DS-160 and print the new confirmation page to bring with you to the interview.

 

ETA: Make sure to update your profile on UStraveldocs website after submitting a new DS-160 as it says to make sure the info on your profile is the same as your DS-160.
