Hello All.

As usual here we are again for help and advce of you all peoples. I land in usa Jan 14 2006 and we got married march 17, And we have our marriage certificate and all other documents which we need for AOS, but the problem is i dont have my Vaccination supplement certificate. I had Medical Test in india and i passed it perfectly but I dont have any other Vaccination supplement with me, which is important for adjust your green card (AOS) case. I live in North PA with my wife and we suppose to adjust our case asap but now we figured it out that we need Vaccination Supplement so can anyone help me how to get Vaccination supplement here? i searched for Local Civil Surgons but i heard its to get Vaccination Supplement here, so can anyone help me what to do and how to get Cheap Vaccination Supplement Report in PA? (I have all my vaccinations in india but dont have any record).

If your medical worksheet (DS-230 I think) states that ALL vaccinations have been given at the time of the K-1 medical, then you do not need a supplemental form. If any are missing, just call around to all the civil surgeons listed from the USCIS website, state you need xxx vacination and the I-693 supplement form and go with the best price.

AOS appointment letter received Feb 3 2005

AOS interview mostly fine, just need I-693 supplement filled out by civil surgeon

Spend another $40 for I-693 form, sent to local USCIS office registered mail, arrived March 11 2005

AOS finally approved March 25th 2005

Green card arrives about 10 days later

!!Green card has incorrect middle name!! Thanks USCIS

Travel back to Denver, turn in card and I-90 to correct their mistake.

Application accepted, asked for I-551 stamp, they told me "We don't do stamps anymore"

NOA1 for I-90 received from NSC April 18 2005, fee waived

NSC appears to have stoped processing I-90's

Marilyn needs to travel back to the RP, call Senators office for help with green card or I-551 stamp

Travel back to Denver July 1, 2005 and received I-551 stamp in the passport.

Do you have your Vaccination Worksheet DS3025 (from the doctor who did your medical in India)? If you do, you can use that to get the I693 filled out. You have to call around to find the cheapest Civil Surgeon in your area maybe a doctor friend of yours (many Indians are doctors) can help you. If you do not have your DS3025 then maybe try and get a copy from the doctor (have a relative contact him in India) if possible; otherwise go to a local health clinic and get all the shots you need and then take their certificate to a civil surgeon. First of all, I thought that there are several people here who submitted just the vaccination worksheet which they received from their overseas doctor (DS 3025, by the way) and they got RFE'd because they wanted the vaccination supplement form I-693a....? Of course there also several people who didn't submit it and passed without problems......so I wouldn't say that the vaccination supplement is not necessary but that everyone has to decide for himself whether he wants to take the risk of getting an RFE or not.

Secondly, as I understand his post, he doesn't have ANY prooof of vaccinations, not even the vaccination worksheet from his country, even though he has had all the required vaccinations (he wrote "I have all my vaccinations in india but dont have any record")

In this case, I would recommend getting a titer test (is that the same in English? Just know it from German, sorry) to prove that he has had the shots and then have the vaccination supplement filled out.

I do agree with the advice of calling around for prices...they differ greatly.

Thank you all for your meaningful advice and help, i appricated it.

But i dont have any Medical Report with me (Vaccination Worksheet DS3025) Cus when i landed to the usa immegration get packet from me, i guess my medical report was in that packet but they never returned it to me and all i have is my X-ray now which i got with my medical report.

So if i get any Vaccination supplement from India then it will be work out here?

Its drving me nuts, lol sorry but i need help right now, thanks you all esp Motu

You need to show you received MMR, Varicella and Td vaccines.
