Check this link (official USCIS) and in the sectiuon: Where to File? You´ll find the guide according to your state and the addresses:

Don´t thank me,is a pleasure to me help others, like you, the picture´s resolution is the highest that I have it, but is just to make a general idea of the assembly!

Please, do a triple check of the entire package before send it ok!
