Employment History Gap, etc.

Hi! Just a quick, overly ####### question haha. I am filing as of this week (squeal) and I have my employment history for the past five years, called and made sure to get dates from my employers, etc. There are three spaces with an option for more. I worked one of my jobs from June 2012-Jan 2015, but prior to this I had a waitressing job briefly, from I don't even remember the dates, March 2012-May? Is it  necessary that I include this? Does my history have to include the entire calendar year for 2012, or is June 2012-present good because that is technically over five years ago?

 

Also, I noticed that the old forms ask to explain any gaps in employment, and these do not. I was going to provide an explanation of mine from 2016, especially since it matches all of my evidence of being with my fiance in Europe last year. Is this advisable? Should he do the same, as he went travelling two years ago when he left the job he was in for ten years? Thanks guys, you're always so helpful!
