Below are the instructions for form I-485 edition 06/26/17:

You are NOT required to submit Form I-693 at the time you file your adjustment application, but may do so if you
wish. Because of the time-limited validity of Form I-693, you may choose to submit your Form I-693 after you file
your Form I-485. You may also submit Form I-693 in person at an interview in a USCIS field office, if an interview is
required. By waiting to submit Form I-693, you may avoid having to repeat the immigration medical examination.

thanks appreciate it :)

If your spouse is in the military and you have tricare prime you can also go to the medical center and get the medical exam there. There you also don’t have to pay any fees which means $500 less to spend. ;)

That is very true. But sometimes, some surgeons would take your word for it, if you could verbally convinced them that you have taken a particular vaccination before. In that case, they'd just tick "vaccination history" in the form. But there are other surgeons who'd insist on the applicant bringing a proof. So, yes I agree with what you said, it is best to visit a good surgeon and then go from there.

...and I believe that some of us do most of the vaccines already - but for some reason we don't have the record in hand to show proof - it makes no sense to do over the same vaccines twice - hence the reason to get the Civil Surgeon to rule out what is already done - and what needs to get done - then as you said take it from there - no one has any money to waste right now.

In order for you not to squander your money or delay your filing, I'd advise that you first visit a civil surgeon first. Then explain point by point what you have in terms of vaccination or if you have even done your medicals from your home country. Let the surgeon tell you what you need and what you don't. Use this link to find one closest to you. https://my.uscis.gov/findadoctor

I wish you all the best.

HI there!  Yes, you do need the medical exam. It is called form I-693

(To avoid unnecessary delays, it is better to do it now than wait till your interview)

In order to apply for a permanent residence, you must undergo a medical examination. The purpose behind the examination is to ensure that you are not inadmissible to the U.S. on public health grounds.

However, you cannot simply undergo a medical examination with your regular doctor. Instead, a civil surgeon who has been specially designated by the relevant immigration authorities must perform the required medical examination. The civil surgeons who are qualified to perform green card medical examinations receive special, ongoing training about immigration issues.

Since you are applying for adjustment of status, you can access the list of qualified civil surgeons from USCIS website(https://my.uscis.gov/findadoctor)

Just enter your zip code to get a list of surgeons within your location.

WHAT TO EXPECT

The doctor will talk with you, review your medical and vaccination history, and give you a physical examination, a chest X-ray, and blood test

During the physical portion of the examination, the doctor will, at a minimum, look at your eyes, ears, nose and throat, extremities, heart, lungs, abdomen, lymph nodes, skin, and external genitalia.

Check to ensure your vaccination records are up to date. These include:

Hepatitis A and B, Influenza, Influenza type b (Hib), Measles, Meningococcal, Mumps, Pneumococcal, Pertussis, Polio, Rotavirus, Rubella, Tetanus and diphtheria, toxoids, Varicella.

You can get all your vaccinations at a public hospital for a very cheap rate. Some clinics even give you for free(without insurance), but make sure you get most of the vaccines before visiting the surgeon. Trying to get them at the surgeons will practically drain your pocket.

CAUTION!   The doctor will give you a sealed medical report which you are to include in your documents; pls DO NOT OPEN the envelope. However, you can request a copy for your records.

thanks for the info appreciate it :) a very good explanation i didn't know about getting the vaccinations from clinics thanks truly appreciate it

You are required to get an exam done. The civil surgeon will complete and seal the form for you.

You should receive a letter in the mail that is yellow telling you what you should do.

My letter said that if you are scheduled for an interview that you must bring it to the interview with you.

If you do not get an interview, they will send you an RFE to submit the required form.

It also tells you not to send it in unless instructed to do so.

Hope this helps.

thanks appreciate it

I just called around to different doctors in my area and found the cheapest price I ended up paying $200 total I had my husband due to blood titers to prove immunity for mmr and varicella since he did not have true vaccine proof and had him get a tetanus shot.  I also had him due he blood test for Tv called quanteferion so he did not have to do the RN skin test (my health plan covered all the costs of the blood test titrea and vaccines fortunately) The exam was straight forward we just brought the filled out papers with along with the vaccine Information.

Filed I-485, I-130, I-765, & I-131 priority date August 9, 2017 spousal AOS
