Below are the instructions for form I-485 edition 06/26/17:

You are NOT required to submit Form I-693 at the time you file your adjustment application, but may do so if you
wish. Because of the time-limited validity of Form I-693, you may choose to submit your Form I-693 after you file
your Form I-485. You may also submit Form I-693 in person at an interview in a USCIS field office, if an interview is
required. By waiting to submit Form I-693, you may avoid having to repeat the immigration medical examination.

You are required to get an exam done. The civil surgeon will complete and seal the form for you.

You should receive a letter in the mail that is yellow telling you what you should do.

My letter said that if you are scheduled for an interview that you must bring it to the interview with you.

If you do not get an interview, they will send you an RFE to submit the required form.

It also tells you not to send it in unless instructed to do so.

Hope this helps.

HI there!  Yes, you do need the medical exam. It is called form I-693

(To avoid unnecessary delays, it is better to do it now than wait till your interview)

In order to apply for a permanent residence, you must undergo a medical examination. The purpose behind the examination is to ensure that you are not inadmissible to the U.S. on public health grounds.

However, you cannot simply undergo a medical examination with your regular doctor. Instead, a civil surgeon who has been specially designated by the relevant immigration authorities must perform the required medical examination. The civil surgeons who are qualified to perform green card medical examinations receive special, ongoing training about immigration issues.

Since you are applying for adjustment of status, you can access the list of qualified civil surgeons from USCIS website(https://my.uscis.gov/findadoctor)

Just enter your zip code to get a list of surgeons within your location.

WHAT TO EXPECT

The doctor will talk with you, review your medical and vaccination history, and give you a physical examination, a chest X-ray, and blood test

During the physical portion of the examination, the doctor will, at a minimum, look at your eyes, ears, nose and throat, extremities, heart, lungs, abdomen, lymph nodes, skin, and external genitalia.

Check to ensure your vaccination records are up to date. These include:

Hepatitis A and B, Influenza, Influenza type b (Hib), Measles, Meningococcal, Mumps, Pneumococcal, Pertussis, Polio, Rotavirus, Rubella, Tetanus and diphtheria, toxoids, Varicella.

You can get all your vaccinations at a public hospital for a very cheap rate. Some clinics even give you for free(without insurance), but make sure you get most of the vaccines before visiting the surgeon. Trying to get them at the surgeons will practically drain your pocket.

CAUTION!   The doctor will give you a sealed medical report which you are to include in your documents; pls DO NOT OPEN the envelope. However, you can request a copy for your records.

I do not have all the information at this point, but I dont this this is correct. If all the exams were done on the immigrant country, he wont be requested for additional ones. That is my understanding based on what I read. Only if you had any issues, or not all the vaccinations in your country that could then be requested again. Let me get some additional info.

Hi Jesusboy, this is very good information, thanks for sharing this - but tell me, aren't there some vaccines that are not age appropriate? There are some people I think who have done some of these vaccines, but they don't have a record.  In this case only a blood test can tell whether or not the vaccines were done - I do believe that it's best to see the Civil Surgeon first so that he can rule out which vaccines not to get - and what is appropriate for your age - then after - get vaccines done at a Public Clinic - take back those reports to the Civil Surgeon so that he can fill out Form I-693. I might wrong, but I think most people got measles vaccines, mumps and Polio already....only a blood test can confirm at this stage I think.  What ya think?

People, read this post and follow the USCIS instructions....there is no requeriment for Civil Surgeon if you did take all the exams and vaccination in your home country.

Here is a post that @yuna628 explained exactly what it is during the K1 process;

"You will have a medical exam as part of your K1 process before you leave your home country. It is advisable to receive the required vaccinations either BEFORE the medical, or DURING the medical. You should be given a DS3025, if all your vaccinations are complete. Keep this for your records. Send a photocopy of this DS3025 with your AOS application after you marry. If you file within one year of the expiration of the original medical, a K1 is exempt from having a new medical. Sometimes, despite this you may or may not receive an RFE for a variety of different reasons I won't explain here. If they should request a new vaccination supplement - at that time you will go to a US Civil Surgeon, you will partially fill out a I-693, and they will recertify and seal the vaccinations in an envelop you must reply with. Sometimes though it can be hard to find a US CS that is willing to do this without doing a medical. If for some reason you do not wish to receive vaccinations before the medical or during the medical, you will also need to seek out a US Civil surgeon (and show them proof of having the vaccinations) to recertify and send this with your AOS. You will still be exempt under the K1 rules, but you will not be exempt from the vaccination portion in that case."

Make sure you understand every requirement, otherwise you will be burning money.

That is my understanding too, providing that you have proof - if not after sitting with the civil Surgeon - he will ask you what vaccines you have done already -  based on your age - he will tell you the ones you need to get done - or the ones that needs to be done over - sometimes the Civil Surgeons dont give most of these vaccines so they will  refer you to a Clinic - you get them done - and take back all results to Civil Surgeon being that he is the one that is filling out I-693.  Clarification is welcome, not to old to learn

That is very true. But sometimes, some surgeons would take your word for it, if you could verbally convinced them that you have taken a particular vaccination before. In that case, they'd just tick "vaccination history" in the form. But there are other surgeons who'd insist on the applicant bringing a proof. So, yes I agree with what you said, it is best to visit a good surgeon and then go from there.
