You can ask for permission to leave for a year, but you will have to ROC in your second year and that could be a problem because you won't have evidence of living with your spouse for 1 out of 2 years. That could get you in trouble. The green card is to live in the U.S.

Second, I don't understand why you want to medical school in the Caribbean (you misspelled that) because you won't be able to practice medicine in the U.S. Also, I seriously doubt medical school in St. Lucia is good enough to allow you to take the exam to practice medicine in the U.S. I know people who have taken the exam and they were in the top of the class from really good programs that were over 3 years long, and it was even hard for them.

Medical school in Caribbean has 3 intakes in a year. Only have to stay 18-20 months for the classes and come back to US to start clinical practice. Over here in US medical school requirements are MCAT, need 90hrs perquisite classes, GPA 3.6 or higher, apply today and classes starts next year aug-sept 2018. Here takes a longer time than Caribbean. As I am CR1 green card holder I am confused to go or not to go on this green card.

I would suggest obtaining a re-entry permit for stays abroad that long, even if you come back to the US for short visits. This will help show that you have not abandoned US residence while you study abroad.

Extended travel abroad will impact your eligibility for citizenship due to the physical presence requirements.

Edit: To clarify, this affects the timeline for your eligibility...you can still qualify once you meet the requirement.

You cannot touch a patient unless you've taken the revalidation MD exam. Have you looked into that?

I'm not talking out of my ###. My dad is a cardiologist and he took the revalidation exam, did his residency and PhD at Stanford, and then went back to my country because he wanted to go back.

Yes, I know that. I have to give USMLE before starting clinical rotation. So, after finishing classes in Caribbean(18-20 month) come back to US give the USMLE test and at the same time I have to apply for Conditional Removal I-751 form. Then start with clinical rotation. This is my planning. But I am worried about my CR1 green card. Would it be affected while on applying I-751 form or create any query on that? I will be staying alone in Caribbean. I will come back and forth every 4 months.

Yes, why do you think doing a 1-year program with professors that didn't study in the US would give you the necessary skills to pass the exam? The exam is very "US developed" with multiple choice questions, etc. You would be competing with people that studied more time and that who were already prepared to do that (the scores, grades from pre-med, etc. that you mentioned).

As I said, you are supposed to do ROC in year 2 and you would be basically out of the country for most of the time so I don't know how much evidence you'll be able to provide of intermingled finances. That is not just having a joint bank account but actually using it. Also, you will be given an interview date and that could be anytime and any day, you cannot really pick and choose unless you have a great excuse (and living abroad is not one).

As someone said above, you can get a 1 year reentry permit. But again, you have ROC.

You can (and must!) file for ROC still. You can file while abroad fine, but you must do the biometrics and interview within the US still.

While proving a bona fide marriage can (and is) absolutely possible under those circumstances, they do like to see evidence of cohabitation, commingling of finances, etc., which may be unavailable or complicated by your situation. Be sure to provide plenty of documentation of having a bona fide marriage.
