My wife's application was just approved by USCIS in Rome (shes italian, i'm American, we've been married forum about 2 1/5 years). I received the eletronic notification last week that our I-130 petition would be sent to Naples by the end of the week. We are just waiting now for them to receive It and contact us. My question is, once they contact us, how long does It usually take to schedule the medical exam/appointment from the time we receive our notification? And, once scheduled, what sort of additional documentation will bè required? So basically, how much do You guys think we have left? I mean, what do you guys think we can tell our families as a loose departure date/arrival date?

I am kind of surprised how your case will be sent to Naples when only the USCIS approved your case!  Usually after the USCIS approves an I-130 case, it takes 2 weeks to send it to the NVC.  The NVC will then take 1 - 2 weeks to give your case a number then you have to fill out some forms online and pay the fees which take about a week.  Then you can send your documents to the NVC and once the NVC receives your documents, then they usually take 11 weeks to review the documents.  Once the documents are reviewed, an embassy interview is scheduled which has to be at least 30 days from the day you are notified.  The NVC process takes about 15 weeks and the embassy is around 4 weeks.  But I am not sure how the USCIS will send your case to Naples and skip the NVC.  Good luck though and good for you if they will skip the NVC

Yeah this whole thing is strange. We did send in our petition on june 1st and The Rome USCIS sent me this on the 25th of july - 

We are pleased to inform you that the I-130 petition for your spouse has been granted today. A notice of approval has been mailed to you and the approved petition will be sent to the U.S. Consulate in Naples (Immigrant Visa Section) by the end of this week. Their contact information follows: IVNaples@state.gov.

Unless i'm mistaken, this means that Naples will contact me to schedule an appointment/medical visit (at least thats what i got from this)

It sounds like you applied from Rome, not the USA.  The process I explained above is if you apply from the USA

When I said 15 weeks, 2 weeks for the USCIS to send the approved case to the NVC and 1-2 weeks for the NVC to process a case number. Then you have to add some time to pay the fees and submit the forms online which you can't do all at once. You must do each step one at a time because the NVC doesn't allow you to proceed to the next step unless the step before it is complete.

The NVC is taking 11 weeks to review documents and thats the timing on their website and a lot of members are reporting the same timing.  May be for a I-129 K1 it's less than 6 weeks, but for an I-130 its around the same timing I described.  Give or take 1-2 weeks

Currently Naples has opening for September/October. 

I got my approval from USCIS Rome on June the 7th and on June 12 Naples sent me my case number and all the instructions I needed to follow. 

After a few days I booked my appointment for the end of August.

Just got contacted by IVNaples. Sent us all the information and we've collected all the other documents they need. Booked our appointment for September 26th. Now I'm really confused with the DS-260 form. We just filled it out and sent it in but do we now need to send stuff to NVC? I can't thank you enough for all your help, really. It's amazing that we found somebody that's going through the same process.
