As far as I know by law, if your spouse have 401K account thru employer then you're the automatic beneficiary @KYUK is right, your spouse can request a letter from employer stating that you're the beneficiary from your spouse 401k.

Fidelity offers free financial planning to anyone who has an IRA or 401(k) with them.  I've had 3 good ones over the years.  If all else fails, make an appointment with a financial planner at the closest local office.  You can get good advice from them, and at the same time, explain what you need for ROC, and ask the planner how you can get a copy.  I have my account set up so that I get all documents via snail mail.  I NEVER access my account online, and when I've been in the office and asked any of my planners to print something for me, they done so happily.

Absolutely. it mentions the name and SSN of the person who the account belongs to.  As for Fidelity, they are a huge company and I am frankly shocked at what they have told you. They should be able to send you a quarterly statement via regular mail at the very least. I have had them in the past and had no issues getting the 1099 as well as statements.

Next time you call them ask to speak to a supervisor and then make your request...I'm flabbergasted that the person answering the phone is unwilling to send you anything.  Sounds like laziness to me.

Lol, thanks for clarifying. It's not me who started the topic. But I think this information might be valuable for the OP and other people who are dealing with same issue.

Unfortunately this happens a lot. First I thought it was because of communication issues, but my husband who is a USC told me that it's very common. I'm not gonna bore anyone with all the details, but I've learned that I need to be persistent. Take a deep breath and approach it from a different angle.

As far as the "joint" credit cards go...You may be an additional authorized user which is different from you be jointly responsible for the charges.  Unless you filled out an application listing you as well as your spouse  being responsible for the account your name will not appear on the credit card statements.  I can add anyone to my Macy's account and get them a card but unless they fill out an app they are not jointly responsible for the payment of the account.  I hope this helps.

Why you need a actual mail. A screenshot from your account which shows you as 401k beneficiary is good enough.

You can not have your name on 401k account statement because you are beneficiary not account owner. Statement will only have account owners name. A screenshot from your online account showing you a beneficiary is good enough.

wow, thanks for all the comments! Well we called fidelity for the 4th time, we had one person say they submitted a request to write a letter to state i am the beneficiary and it may or may  not come..in the meantime i have a screenshot from the online portal and one statement showing the funds which is under my husbands name. I will update everyone on this when the letter arrives/doesn't arrive.

 

As for the credit card statements, they do not show any numbers on either purchases, so still trying to figure this one - probably just going to show the two cards under both names,  and a few statements even if they aren't under my name. not much more i can do!

 

We have a pretty good amount of additional evidence, a home title, car insurances, home insurance, health insurance, bills, wills, and debit and savings accounts so i think we will be ok?!
