Filipino beneficiary worked as OFW for two years, she wasn't able to get a police clearance while there, they did give a identity investigation report showing entre and exit visa in good status and resident permit was good, and fingerprints were on file. I am worried she cannot get an actual police clearance from Saudi, is this going to cause a big problem? what to do ? will the US embassy reject this when she has a background check?

My Filipino fiancee recently went through this as well. She worked in KSA for several years then exited and returned to PI. She interviewed at USEM a few weeks ago and was approved.

At the interview, they asked for her NBI clearance, but not the KSA police certs. This may have just been an oversight on their part...I don't want to set the impression that they will not request KSA police certs. But they are difficult to come by...usually impossible if you no longer have a visa to enter the country and are not a Saudi national. The reciprocity schedule says "Police clearances are available for Saudi citizens and resident third country nationals. Saudi citizens currently residing overseas must return in person to process the police clearance.". It also says " Exceptions:  Police clearances are unavailable for third country nationals who no longer reside in the Kingdom.". As such, there is not a technical requirement to present one once residency is terminated.

That said, it's not uncommon for many embassies to still request the police cert, or to provide evidence/a statement that they cannot be obtained. So I don't think it will prevent your fiancee from obtaining the visa, but it may delay it slightly if they request it and you need to demonstrate that you are unable to present it.
