Help me out----so anyone here use a doc who faxed the info to you or sent it? I am willing to pay I don't care just to get the stupid thing done. Who did you use and where.......i am close to raleigh NC and called some on the list no luck yet with anyone faxing it . HELP before i turn to my own doc for medication for crying out loud.

I don't think a Civil Surgeon will do it by fax. Just use the locater and enter 27602 for a Zip Code.

So what if I faxed the info and then they sign it and mail it to me? That would work, what a pain in the behind......

I don't get it. According to your timeline, your AOS process is complete and you have your green card... why are they asking your for evidence now ?

I agree with the other posters. Dr.'s won't just fax that form off, it needs to be filled out by the DR. and then put in a sealed envelope. These Dr's have guidelines laid out for them, and they follow them for that reason. My Civil Surgeon had to give us a sealed envelope. He didn't even give us a copy of the document just the sealed envelope. You'll have to see someone personally, and show your evidence and get the suppliment filled out that way.

AOS:

2007-02-22: Sent AOS /EAD

2007-03-06 : NOA1 AOS /EAD

2007-03-28: Transferred to CSC

2007-05-17: EAD Card Production Ordered

2007-05-21: I485 Approved

2007-05-24: EAD Card Received

2007-06-01: Green Card Received!!

Removal of Conditions:

2009-02-27: Sent I-751

2009-03-07: NOA I-751

2009-03-31: Biometrics Appt. Hartford

2009-07-21: Touched (first time since biometrics) Perhaps address change?

2009-07-28: Approved at VSC

2009-08-25: Received card in the mail

Not for me for the kids filed under my K1 and they are taking their sweet time to get me the cards...I've had mine for 2 months, they are actually supposed to give any petitions under a k1 all at the same time so this blows. Oh well I have a appt on Wed for the kids then I will send the rfe asap! SAd, don't even care about mine, just theirs for travel!

Copies can be altered, I suspect the doctor will only accept the original documents in hand, a trip to the civil surgeon is needed.

Oh well I have a appt on Wed for the kids then I will send the rfe asap! SAd, don't even care about mine, just theirs for travel!

Did you ever send their medical with the AOS package ?

A copy of the immunizations along with mine...did'nt ask for me....just them nor worries heading to get the I693 onm Wednesday then send it back.....hoping they move thier butts and send the card after that....we'll see, whats the time line for that?

The OP sent a copy of the vaccination worksheet from the medical abroad, not a I-693A (same info, different form) which is what the RFE is asking for but only for the kids.

Then why is the OP so upset and angry, acting as though the USCIS is making a request out-of-this-world ? I'm sorry but If you are supposed to send something and you don't (intentional or not) and you get an RFE, you have absolutely no right to be mad.
