Hi all,

 

We are preparing to submit concurrent filing of I-130 and I-485. And while finalizing documents, we realized that our i-693 vaccination record (on the copy) has an issue -- it is missing a check mark on the very first row for DT / DTaP / DTP (it should've been marked Not Age Appropriate).  The record does show completed mark with date for Tdap, and also the results section is marked on "Application may be eligible for blanket waivers".

 

In theory it may be ok but we are worried since 1) the instruction says "Please make sure to mark every row" 2) we've seen a case of RFE due to missing a check mark here. 

 

Could anyone please share thoughts if we should re-do it with Civil Surgeon or if it is general practice to miss the mark for DTaP?
