I guess I don't understand the question.

If I understand correctly, you work as employee for one company where you receive a W2 and pay check, and another company where you do not receive a W2, and have a contract to do work for company?

When I think someone is self employed then they have their own business address and company name.

If this is the case, I would write in the name and address of the company where you are employed.  You can also include the self employed company you work for in part 7 Additional information.

Or you can write a letter explaining your sources of income.

I work for a company and also teach part time for a local community college, so I wrote a letter explaining both sources of income, and I have pay stubs and W2's for both.

You will need to prove the self-employed income,

thank you for the answer

by self-employed I meant I work for someone and I do my own taxes, I do work as a personal assistant an dI have pay stubs.

and I do work as a nurse in a clink but my employer do my taxes.

so in this case I should put both employed and self-employed and in adress I will put the adress of my employer. an din additional information I will put the adress of the person I  am working for as a self employed.right?

and in annual income I should merge both income and give a numer how much I make from both jobs right?

both jobs are new for me after beong unemployed, so is it ok to apply for fiance visa interview and our petition is going to expire in october and we dont have much tim sinc eI start working . will this affect our case? Ihave a letter of employment, it is a permanant job but only one week pay stub, but also some pay checks for the other job.

can advice me please?

You are not self-employed in either job.  Self employed means you have your own business and charge clients/customers for your services.

Report both jobs on the I-134 as I described.

You want to have employment letters from both employers, and use the income from both.  If you are currently permanently employed and meet the minimum income requirements then you should have no problem with the interview.  By the time you get to the interview you will have a few months of employment history and pay checks to prove this.

I understand your answer but Iwant to be more specific with personal assistant job.I am not hired through an agency but I work directly with an elderly, so I work for this elderly person and I take care of na dhe in return pay me for the duration I worked for.in this job I mean personal assistant I do my own taxes.

please , no offense in my answer I just exchange my situation and info with you for better understanding and for help.  in this case I see that this job is considered as a self-employed since I do my own taxes, but may be I am wrong I need nore clarification for this point to fill out the I-134 please. and how immiragtion laws look to difference between self-employed and employed>

thank you
