Another Mistake on G-325a....HELP!!!

Timeline

My mother's middle name or surname is not listed anywhere in her official documents before marriage. She only has first name as her maiden name. I didn't know that and wrote her surname in G-325a according to her father's surname which is exactly the same as her current husband's surname that she took after marriage. So i haven't exactly mentioned anything wrong it's just not listed anywhere on her documents. What should i do? My petition has been approved and I haven't filed for my ds-260 yet and want to send it correctly. Should i still go with the same surname in ds-260 or should i leave it blank or n/a or what? How to explain it at the interview? I am extremely worried about the discrepancies. Has anyone else had two or more mistakes on their forms due to human error and there were no consequences? Is this seen as hiding something? Please Help!!!

I don't see this as a problem, or think it is a mistake.  The form doesn't ask what name is on her documents.  It asks for her maiden name.  Sounds like you answered correctly.

Thank you for responding. Makes me feel much better. Also she took my father's name after marriage so should i mention that in the given names in Ds-260 or only her maiden name?

From what you wrote, her father's surname and her husband's surname are the same, so she has used not "other" surname.  For example when Mary Jane SMITH, marries John SMITH her name doesn't change.  Mary and Jane are the given names, with SMITH being the only surname ever used.  Have I misunderstood your mother's circumstances?

Yes she hasn't used any other surname. The surname is same before and after the marriage. My mother had no middle name either before marriage. My mother though took my father's middle name too along with the surname. So should i mention that middle name that she adopted after marriage or just mention her maiden name without the middle name. Also on my mother's birth certificate it only says her first name. No mention of her surname. So i am thinking to get her birth certificate and documents changed so she has proper documentation. I am just worried about getting affected because of this.

Hey. I just wanted to know that since you said i should follow the name she was born with then her name according to her birth certificate and all documents is JANE while all her family members are JOHN SMITH, JILL SMITH etc. Since i wrote on my g325-a form that her surname is SMITH and her first name is JANE. Would it cause a big problem since i have no documentation to prove she was JANE SMITH before marriage. Should i get her birth certificate etc corrected? Or do you think NVC will be able to figure out her name without giving me a RFE. I am still reluctant to submit my ds-260 because of this. Or should i mention in her given names slot as only JANE. I don't want to make any more mistakes.
