I-130 Part 5 Questions

I have a question about Part 5. I was previously married to someone else and we had gone through the K-1 Fiance visa. It was approved no problems etc. 

Question 1 asks if I ever have previously filed a petition for this beneficiary or any other alien? The answer is obviously yes.

But, the next questions are what I don't know:

Question 3.a. asks for the City or Town

Question 3.b. asks for the state.

Do I put my city and state or the city and state of the service center? 

Question 4 asks for the date filed. I have the receipt # etc I looked it up online I am not sure what the filing date is. I looked through all my old paperwork and I can't seem to find the answer. I know it was in 2011. I know it was approved on April 20th, 2011. But I do not know when the filing date was. I saw somewhere that I can tell the filing date from the receipt #.

The receipt date will do nicely as the filing date.  If you don't have it, hand write "about" next to the best date you can give them.  Where filed is not critical as the petition would have been sent to a PO Box in Dallas for the last several years.  That or the service center will both be fine.
