I notice that other ppl are sending in their medical with these forms but I was told that since my husbands packet was taken by the POE in Chicago ( that contained all his medical) that we will not be getting it back so due to that fact, we are not required to send in that medical I693, is that right????

He should get a vaccination supplement only, form I-693A, filled out by a civil surgeon in the U.S. He does not need a full medical. There's been a bit of discussion about this past week. Just look for "vaccination supplement" in the thread titles.

Well right now I'm facing the same problem....my Medical Papers was taken away at POE in Chicago which was inside the envelope. I submitted my Vaccination paper with my AOS file. Right now my interview is on 17th April, so got confused with I-693 Medical form. So everything is in God's hands....no worries,all will be great. Trusting Lord

Don't worry about the brown envelope that was taken from you at POE Chicago, they send it to the place where you're going to have your interview. Sending I-693, you mean you took another full medical? What you really need is vaccination supplement from a designated surgeon from the Homeland Security in a sealed envelope. Anyway, I want to wish you goodluck on your interview.

Everyone's brown envelope is taken at the POE. Nothing to worry about. As long as your medical is less than a year, you're fine and all you'll need is the vaccination supplement...the I-693A. Call around on the list of CS's in your area. Some will try to make you believe you have to have a new medical. Trust me...it pays to call around. :)

well i think that is all we have is the vaccination supplement not a I693A- he had his medical in jordan and whatever form they filled out was taken by the immigration at the airport. so maybe all i will send in is a copy of that vaccination report. His medical was at the end of February 2007 so I guess i will just have to wait and see if i will be asked for additional information.

YES mybackpages that is exacty RIGHT! i checked through my husbands checklist from the embassy and that is the form the medical examiner did and at his POE they took that from him. We have no I693 to submit for AOS/EAD filing. I was told that people at the POE take his medical and send it to the appropriate place and that we do not need to worry about that but it still scares me, I do not want to get an RFE for medical but i cant submit something that i never have received. I can only see what will happen. Thanks!!!

Just to verify (since we're going to the doctor's walk-in tomorrow) - we only need the I-693 A (vaccination supplement), right?

The I-693 must be for the HIV test, right? That is what I gathered from talking to the secretary today.

The supplement the I693A is the vaccinations and immunizations if you do not have the form from your home country (the DS-3025?) my suggestion is to go in with all your evidence of immunizations to a CS to have them transfer on to the proper form. (Don't forget to call around for the cheapest price for this task alone if it's been within a year of your medical) Not sending anything might very well get your package sent back to you. The I693 is the physical that the doctor fills out including HIV, TB other communicable diseases, any disease processes that the "patient' may have including mental and also physical deformaties.

Perfect Kristina.

I just wanted to verify this information. My fiance does have the DS-3025 he received from his medical exam last month in Brazil. I wanted to be safe and send the I-693A - so we'll do that this week.

He still has his HIV record along with his actual vaccination card. We'll take everything with us. CYA :thumbs:

That's probably a good idea... I know that the vancouver CS in canada that did my DS form had not put all the information on his sheet, so it was good that I had taken my immunization record for the CS here to complete my form more fully.
