How do I fill out the 1040x??? I'm changing from single to married filing separately. On the top column in spouses name it ask "if a joint return spouses first name and initial" so do I put my wife's name or leave it blank because I'm amending it to married filing separately. Also for the spouses social security number do I put NRA or leave it blank if I'm not suppose to include name.

If your spouse is in the US it is my understanding that you file married filing jointly until your spouse becomes a US citizen. Then you can file separately or continue to file jointly. I say this because i see that your spouse's visa has been approved and you are still within the tax year. Just consult with a tax professional to be certain.
