Purpose:   For travel abroad

Only need one NBI with an AKA for other names used.

I see. So the format above is okay already.

No need for me to get 2 different NBI's, with which the:

1st set is Maiden name, Status: Single, AKA:married name &

2nd set Married name, Status: Annulled, AKA: blank.

Am I right?

Correct.  As long as both names are shown on the NBI you are good to go.

Hi etten it should be

Your current maiden name

Then in your AKA it should be your previous married name and also you need to still fill up the name of your ex husbands on the form......i only got 1 NBI CLEARANCE in that same format....you should not leave any blanks on the clearance.....

I see. I am still in the waiting game for NOA2. I have heard being put on 221g those who were Annulled because of their NBI. That us why I asked, so I can perpare my NBI correctly and would not be put under 221g.

You're a big help. Not confused anymore. 😁

Thanks!

And also bring with you your annotated null and void marriage certificate AND CENOMAR but sometime i guess depends on the CO they will request direct submission of marriage certificate from PSA to USEM those apllicants that undergo annulment process....it happened to me as well i had my MC on my interview day but they requested it so still i was put in 221g....they look for 3 documents that day NBI WITH AKA then DIRECT SUBMISSION OF MC WITH ANNOTATED NULL AND VOID last XEROX OR SCANNED COPY OF BIOGRAPHIC PAGE OF THE PETITIONERS PASSPORT WITH ENTRY AND EXIT STAMP.....this might help you to be prepare on interview day...

Thanks @Hank. Do not want to waste time. I have my MC annotated null and void as well. I am praying that I will be assigned to the CO who is considerate and not strict. That the USEM will not require direct submission of MC (221g again).
