Guys my fiancee has been lived in the last five years in two cities than her current city now so in the question Is your current mailing address the same as your physical address should i choose NO ? and if No i cant write the period date time to becuase it written PRESENT what should i do ?

If the answer  is No ,  provide  her current  physical  address  where  it asks  Date From and Date To, they expect  her current  physical  address  to show the date she started  to live there till to PRESENT, so at this point  the field of Date To will print PRESENT  because  she still lives  there.

Then next address  section  provided

You will  state the date she started  to live on that particular  place and ending  date, so here they expect  to see both dates "Date From and Date To" also be smart  to make these dates correlate, I mean the value at "Date From" at current  physical  address  should  correlate  to the value at "Date To" at the previous  address  she lived before she moved to her current  address, these  dates might be the same or differ in a single  day

Current  Address  

Date  From: 01/01/2017

Date To: PRESENT  

 

Previous  Address  lived before  current  address  

Date From: 01/01/2016

Date To: 12/31/2016

 

If the space  provided  doesn't  respond  when you type  it in, you can fill it by your hand with a black  ink  pen once you print it for signing
