Yes you'll be able to get them when you get here but you do not want the stress and financial headache of getting an approved panel surgeon to sign off on them.

 

Immigration is stressful enough, you want to do all you can to make it as simple as you can.

although I can't help feel that I've left it too late because when I had my most recent two.. I got the WORST cold ever and I don't want to go to the medical with a cold or weak lol

Also, I was in a car accident and broke my collar bone in 2013 and have a small metal plate on my collar bone...

 

do I need to tick the box about being hospitalised? Orrrrrr not?

You need dates for those shots.

 

Your requirements are 

1) Two MMR shots, which you have listed as-

  • Measles/mumps/rubella vaccn 
  • Measles mumps and rubella booster vaccination

2)Tetanus/diphtheria/pertussis with one in last 10 years, which you have listed as-

  • First DTP
  • Second DTP
  • Third DTP
  • Booster DT
  • Low dose diphtheria, Tetanus, and inactivated...

but one one of those last two needs a date within the last 10 years

 

3) Varicella

  • Tell them you had chickenpox to get the jab waived

4) Influenza

  • It's not flu season so it will be waived. Make sure they code your sheet "not flu season"

 

You're good as long as you have dates printed by those shots and one of those in #2 is dated since 2007 (within last 10 years)

As Wuozopo says above, your GP surgery should be able to give you a print out with dates for the vaccinations.

 

If you were in overnight ( I'm assuming you were) tick YES.

I have all the dates etc it just wouldn't let me upload a bigger picture! So looks like I'm all set and ready! Whohooo

 

I will tick the hospilized and take a statement of my surgery 😊
