I sent one for the "package" here is a copy (it has the old fee and some items specific to our application) but gives you an idea

To Whom It May Concern:


Enclosed please find Form I-485 Application to Register Permanent Residence or Adjust Status,
Form I-864 Affidavit of Support, Form I-131 Application for Travel Document,
Form I-765 Application for Employment Authorization and all supporting documents.
My eligibility is based on admission as the fiancée of a U.S. citizen 
and subsequent marriage to that citizen within 90 days of entry.


Contents include:

- Money order: Payment in the amount of $1070 (Biometrics fee included)

- I-485 Application to Register Permanent Residence or Adjust Status

- G-325A, Biographic Information Sheet

- I-131 Application for Travel Document

- I-765 Application for Employment Document
- Copy of Form I-797 Notice of Action (Approval Notice)
- Copy of international Birth certificate (“civic registration certificate” with certified English Translation )
- Copy of passport, biographic pages, nonimmigrant K1-visa and entry stamp pages
- Print-out of electronic form I-94
- Certified Copy of Marriage Certificate

- Copy of Divorce (from previous marriage) with certified translation (“ultimate order of the court”)
- DS-3025 vaccination documentation worksheet showing US vaccination requirements COMPLETE
- Six (2+2+2) passport style pictures

- I-864 Affidavit of Support
- Federal income tax return, including W-2’s for the year 2015
- Letter of employment
- Copy of pay stubs within the last 6 months
- Copy of US citizen’s passport

- Copy of first page of Apartment Lease as proof of joint residency


Copies of documents submitted are exact photocopies of unaltered documents and I understand
that I may be required to submit original documents to an Immigration or Consular officer at a later date.
