Hello guys, Am gathering all my papers for AOS and I have questions regarding my DS-3025. On my DS-3025 (please see the pics) many of the options are answered with "A" and when it comes to MMR, Varicella and Flu...it just shows nothing. Am not really concerned about Flu shots because its not the flu season. Do i have to get the MMR and Varicella shots and get the details transferred onto I-693 with a USCIS approved civil surgeon? Also, on the page two, its says "Vaccine waiver given in view of K1 visa category". My K1 visa went smoothly without any RFEand i want the same with the AOS. Your DS3025 is not marked as complete, you cannot use this one for your AOS package. Your choice is to get the vaccines transcribed or wait for an RFE at a later date. Your medical will most likely be expired by the time they get to your file so there is a change they will want you to take a new medical.

But the MMR and Varicella section is blank on my DS3025 and will the CS transcribe the same onto I693? or do i really have to take vaccine shots and then go to CS ...like i said i dont want a RFE...i found a USCIS approved doctor in CIncinnati who takes $50 to transcribe

IF $50,= is worth your piece of mind, than do it. You may want to check with them on the shots, but unless you can proof you've had them your likely to be required to get them.

 

Also take into account the time you spent on getting this orginized. If it takes a couple of weeks to get everything set up, it might be worth just sending it in and wait for a potential RFE.

 

The way I read the forms, you are not required to sent in anything on the medical and just hope the paperwork in your current file is enough. Most people sent in the DS3025 (if filled out correctly) as a "friendly" reminder.

I thought you shouldn't open the sealed envelope?

This is the copy given to me by the Panel Doctor in India. USCIS says Panel Doctor should give one copy to the applicant and the other to be in a sealed document.
