You did not have chicken pox as a child?

MMR is a 3 in one vaccine and so is the TDAP, so that is 6 different vaccines combined. The chicken pox makes 7 and the Flu makes 8. So you got 8, not 15 -_-

That's what the doctor asked me, I said I never had chicken pox and he confirmed it from my mom as well.

Timeline

If you are getting a PCC from India please make sure it is issued by the Regional Passport office and not by the Police Commissioners office. I made this mistake and they asked me to submit the correct one and wait for another 11 weeks. There is no option to bring this to the interview directly as you will not have a CC till they have reviewed the correct PCC. Happy to answer any other questions you guys have.

Yes it should be issued from PSK... Did you get a stamp in your passport along with PCC document ?

  1. I had read an experience by an Indian VJer ( can't find it now) that he too received a checklist for Birth Certificate. He did not have his, and hence only submitted his matriculation record. NVC came back - he was then asked to submit a record from the authorities that his Birth Certificate was not available. In your case only submitting the matriculation record MIGHT end up in the same problem.
  2. If you submit the Matriculation record as well, the NVC would understand that the name change is not recent and the record is genuine. I am sure you are having all your records translated in English (as you already suggested). Good Luck.

I got a few of my documents translated from Hindi to English and followed the NVC guidelines for translation. I got the translations notarized alongwith an affidavit from the translator asserting his capability to understand and translate from Hindi to English.

 

That is how I went about it since I feel that extra work does no harm.
