I'm trying to make sense of the additional page of I-30 form. I understand  Page Number, Part Number, but Item Number? For example, let's say address. Page Number 2, Part Number 2, but what do I put for Item Number? 12 a to 12 e? I've been looking for a sample of I-30 form additional page but I only found an outdated form that was pre-I-30...?? Anyone can help out with that??

If its about one certain item (let's say you want to clarify your address you have marked under 12, then item number would be just "12".

If you want to add additional address, then item number would be "10-15", as addresses go all the way through 10 to 15.

In I-130A for extra addresses item would be "4-7".

At least I found that possibility when I was researching what other people have done.

What I did was

page number : 2   Part number:14  item number: A

then I put the address in the same format and location just like it is on page 2. I did it for both of my additional pages and they didn't care. It's been approved without any RFE's. Just like how you would write it on an envelope. Then with the FROM and TO just write

FROM:date

TO: date

make sure it's the same format!

 ALSO don't forget to sign and date each additional info. Hope it helps

Hopefully you're looking for the i-130 not an i-30....

page = page of I-130 form example: 2 (page 2 of 12)

Part = part of the form: example: 2  (Part 2. Information About You (petitioner))

item = entire item number sequence: example:  12a-13b (addresses)

In the open space available, simply type additional residence history etc. and then continue with the information.  Let them figure out what page it is, if they think they really need to.

You are referring to an I-130 form.  The same applies to the I-130a.  I-30 is not a meaningful abbreviation but we know what you mean.
