Documents for Medical in Guangzhou

Could anyone here please tell me what specific documents are required at the Health Care Center of Guangdong International Travel? I have the passport,, appointment letter from NVC(electronic processing), 8 passport photos. Stepson has no shot records, and they keep mentioning a " form with form number" but give no indication of which form or number?

My fiance just went thru the process in Guangzhou. She brought her passport, passport pictures, her interview appointment letter and her DS-160 header page. Maybe the item you are referring to is the DS-160 header page. 

 

Also, note that they take Urine samples and will reject the appointment if your fiance in on her Period at the time of the medical exam.
