Hello,

 

I would like to know if there is anybody here who was only the city of birth mentioned on the passport and not the country.

If there is any, I would like to know if there was any other papers submitted along with DS11 to clarify the request.

You can try but it very rarely gets approved. I wouldn't try this in a post office - go to Special Issuance Agency.
