Once I receive a KI visa can a request that the expiry date be extended to a later date?

Generally, no. In certain circumstances, you can request for the embassy to issue a new one, but they have no obligation to actually do so. A new medical will likely be required since the visa's expiration is tied (by 6 months) to the date of the medical or the date of the interview, whichever came first.

You can have the validity of the approval of the i-129f extended though, so you can push your interview to a later date. The i-129f approval can in most cases be extended 4 month at a time up to one year at the most. 

 

But like geowrian says, once you've had your interview and you're approved for the K-1 there's not much you can do.
