Well now I am really worried. My husband didn't get the "official" DS5535 form, in fact, he was given a photo copied unofficial letter that basically has the same questions but not as extensive as the official form. Nothing asking about relatives, only the names of any siblings/children/partners not recorded on the DS-160. :( He listed none....

it was so confusing what they gave him. I hope we gave them the information they wanted...

Actually the ds-5535 only ask for siblings names. It’s a replica of ds-160, just not as long

Ok i got 10 questions via email but not a form, then i came to know that it is DS-5535 questions. We can only wait now :(

Maybe because the original form was expiring? We sent all the information they wanted to the embassy's email address they had written on the form. I mean I couldn't even read who the officer was who gave it to him.

I wonder if they have come to see that the form 5535 doesn’t provide enough room for some people to write in 15 years worth of information.  If that is the case I imagine they will modify the form in the future. For now maybe they are just giving you the questions in this format so you have plenty of space to write in your responses. I know my husband had a few additional pages he had to create and add to the original  form due to spacing issues.
