i know you are frustrated but in your situation there is things you need to consider 

1 - Islamabad embassy it count very busy

2- the kind of visa you are applying its f-4 (( thats mean they have a cir-ten number ti issued every year ))

3- AP some cases around 20% of them will take around 3-6 month you are 100 day thats mean around 3 and half month 

so hold on its coming

Em so depressed...my all family gone USA tomorrow with the same visa category

So why many r waiting so long.  More than 8 weeks???? I hear we hv to wait 60 days....im 50 days.....but im wondering why many took much more than 60 days

AP its a black hole .. each case is unique and they take it individual ,, but the big percentage only take up to 60 days .. the cause of that travel ban red flags not bonafida marriages .. you name it

Yeah... VO approved my visa at the time of interview and he did not give me any 221g... He asked to pick passport in 4 days with visa stamp. But then after 1.5 month I got form DS 5535 in email. And after that I got nothing yet... Time frame is different for each n every person

Now I am not sure wether they are sending passport for medical or sending her visa.

her medical is going to expire in 20 days...

What was the name of the questionnaire? What type of questions did it ask for? When did he receive it.
