I don't understand what you mean by this .

Just use this site, Go to Guides on this site, then you will samples of form filled out here...just change the information on the examples to yours. Also make sure u download the current form from WWW.USCIS.GOV.

then use the sample here to apply. If you apply early, before the expiration, then you are covered.
