My wife currently still has her maiden name from before we were married. She plans to change it to mine after we do her N-400. Will there be any issues with ROC if your spouse retains their maiden name? Thanks.

No, spouses never have to change their names, no issues for ROC.

Quick question - is her decision to wait based on the fact that she can't change her name on her Italian passport.

That's good to know!

She has a Serbian passport and had Italian residency before we were married. In Italy spouses keep their maiden name and we were concerned that would cause issues if she changed her last name too soon.

Ah, interesting, thanks! It's the reason why I'm waiting for my N-400 as well, can't change my name in my Brazilian passport. But I did change it in Massachusetts.
