I have been reading some posts on I-693 for AOS. My fiancee has got a vaccination supplement from her overseas medical appointment but this form does not have anything like I-693A mentioned on it. I was wondering if I am missing some form or have the rules changed? I also wanted to confirm that do I have to submit a photocopy of this vaccination supplement or the original copy.

The medical done for fiance visas over seas have a vacination supplement but is a DS form not a I form. When filing for AOS you are required to have a civil surgeon complete a I-693A (medical supplement) which is then submitted with your application. If you medical for the Fiance visa was less than one year ago, then you do not need the I-693 only the I-693A.

What you probably have is a vaccination worksheet. I always forget the number, but I think it's called a 3025. Regardless, what you do is one of two things:

1. Take your chances and submit the worksheet with your AOS petition. Some get RFE's asking for the I-693a ... some don't.

2. Take your vaccination worksheet to a local civil surgeon and get them to transfer all that info to a I-693a, which is the vaccination supplement. Call around for costs first as though most of us pay between $15 and $30, some have been known to pay $75 and more just for this simple bit of paperwork.

I'm sure you will have people weighing in on the pros and cons of getting the I-693a instead of using the worksheet. Me? I'm a worrywart - I got the supplement done.

Call around to get a decent price (ranges from $10 - $50 or so) and if you have had the medical in the past year you DO NOT need a full medical again, some will tell you that you do.

You take your vaccine sheet you recieved at the medical in your country with you , they give you the form in a signed, sealed stamped envelope to put in your package.

Some get RFEs for not sending in an actual I-693A and some don't. I didn't want to chance it so I got mine done today. I didn't bother calling around, just the first guy that I talked to since he was close by and could slot me in right away to do it.

I took your advice and called up 3-4 physicians from the list and all of them (or their operators) advice that you need a full medical examination and do not understand what a I-693A form is or what an overseas vaccination supplement is. Did you guys use any special terms to make them understand?

All I did was say something like, "I need a vaccination supplement filled out. Form I-693A. I don't need an entire medical as I arrived on a K visa and had my medical in Canada. How much for just the supplement and when can I come in?"

The doctor told me to bring whatever I have showing my vaccinations were current so I brought the supplement from my Canadian medical as well as documentation from my family doctor (that I used also at my medical).

What you have is not an international/overseas supplement. You have a vaccination worksheet from the civil surgeon, authorized by the US to perform medicals for people seeking US visas. Show them the guidelines on the I-485 that clearly state that you DO NOT need an entire medical if your medical was done in the past year.

The office didn't know exactly what I was trying to ask for either until I called it the medical supplement form for applying for a green card through immigration.

One office I called knew exactly what I wanted the other was cluseless and didn't understand. Try again or shop around some more, stress what you need and if you have to take the instruction page to the I-485 to show proof and your vaccines and even if you have it, a receipt from your orgional medical (I have one ).

We sent in the vaccination supplement done at the foreign medical. We were going to get the I-693a done locally but all the offices wanted to do the full medical. We decided to take our chance and go to the AOS interview without the I-693a thinking that in the worst case they would make us do it and there would be a few weeks delay in finishing our case. Turns out the officer that conducted our interview said the supplement was fine. Got the stamp in the passport that day and green card in the mail a few weeks later.

I'm so glad that I found this thread. We have the DS-3025 form from my fiance's medical doctor in Brazil. He just had his medical done last month.

I was going to send a copy of that - but started to worry as I read about the vaccination supplement.

It's worth it to call the civil surgeons around me, but I want to send this AOS and EAD off as soon as we get married and NOT pay $1000 to do it - so I guess the choice is ours.. Thanks everyone.

i just wanted to say that my husband is from morocco too and all we sent was the vacc. supplement worksheet the DS-3025 and no RFE. dont ask how that happend but i didnt complain...lol... all though we will double check and make sure that his vacc. are up to date and see if he is missing any cuz i don't have a clue... shhh... dont tell anyone!!!

This debate has been going on for over a year and a half. It seems hit or miss as to whether people get RFE's. We chanced not filing anything (my husband didn't get a copy of his vaccination worksheet from his medical). We put in a letter stating the information wanted was surrendered in the brown packet surrendered at the "this" POE on "this" day. It worked for us, thankfully, as funds were extremely tight at the time. For others, they had to go and get it done.
