We are trying to put together the AOS packet and it says, "

I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy)."   We were wondering where do we get this because the guys at the Port of Entry took all the things in the sealed packet and the Doctor says he can't send them.

During the overseas medical did the foreign fiancé receive a copy of DS3025 from the doctor who performed the exam showing the vaccinations on the record?

 

Sending a photocopy of this form generally is accepted. It does not guarantee an RFE however (due to an uninformed officer, missing/lost paperwork etc) but is an acceptable form of proof. In the event an RFE is received the couple can go to a US civil surgeon who can transcribe the info from the form onto the I-693 (it will be partially completed). In the event that you are missing vaccinations still or have no proof or copy of a DS3025 then you will likely need to go to the US civil surgeon and have the medical over again with the appropriate vaccinations (or you can also go to a pharmacy or clinic and retain proof of receiving the vaccinations and have the US civil surgeon transcribe that to make it cheaper).
