I guess you are referring to part 7 Acknowledgement of Appointment. I was confused by this but from what I understood you just print your full name in that box and then sign further down where it says petitioner's signature. Infact sometimes that box is prefilled out with your full printed name from what you entered in previous sections depending on how you opened the I751 file.

In Evidence of the Relationship section point 5 : 

 

Affidavits sworn to or affirmed by at least two people who have known both of you since your conditional residence
was granted and have personal knowledge of your marriage and relationship. (Such persons may be required to
testify before an immigration officer as to the information contained in the affidavit.) The original affidavit must be
submitted and also contain the following information regarding the person making the affidavit: his or her full name
and address; date and place of birth; relationship to you or your spouse
, if any; and full information and complete
details explaining how the person acquired his or her knowledge. Affidavits must be supported by other types of
evidence listed above.


I have affidavit of two people but in the affidavit there date and place of birth is not mention.

Is date and place of birth of person making the affidavit is mandatory ?

You need to file for Adjustment of Status (AOS). If you are approved you will receive a 2-year conditional GC and only after the 2 years you have to apply for ROC.

Well, it's hard to tell for now... When we filed for I-129F, it got approved withing 3 weeks. Now it takes up to 5-6 months. AOS - was 5-8 months, mine took 15, and what's now - who knows... You can file as soon as you are eligible to do so. Most ROC cases are siting "in line" and untouched for months, waiting for IO to get it done. Plus - ROC is not a priority for USCIS.  So, sometimes your pending N-400  application can "push" USCIS to take an action on your ROC  - because, yes, it must be adjusted first anyway.... unless, of course, there is a pending  background check - in this case nothing will "push" , just have to wait until the background check is over...

My marriage is more than 2.5 years.

 

I already have 2 years conditional GC that will expire in October 2017. I think i have to apply I-751 and that will be for 10 GC ?

 

Adjustment of Status ( AOS) is not applicable for me I-751 have to file ?

Yes, you have to file for ROC as well. (for 10 years green card)

 According to this thread, if you are married more than two years when you enter the US, you don't have to to RoC. See this link below:

So i am married from last 2 years 7 months.

 

When I entered the US till now my marriage is 1 year 7 months

 

Which form i have to fill  I-751 OR I 90
