Timeline

My fiancée got her K1 and arrived into the US about 3 weeks ago. I'm ready to file AOS and came across the need for the DS-3025 for the vaccinations. She didn't need any vaccinations or anything like that and the medical was done just a few months ago at the end of January. Her scanner wasn't working so we couldn't get a perfect copy of the one that was to be submitted with all the other documents to the embassy but we do have a very good picture of it. When looking at the picture I noticed at the top it says to give the applicant a copy. We never got a copy of that form. Do you think we should submit the photo we have? Or are we going to have to find another physician to fill out another form for us? Another option we've thought of doing is calling the place she got her medical done to ask them if they can send us a copy. We have the proof she got her medical done there and the photo of the form. I know they might not but it's worth a shot. Anyone have this happen to them or have any advice on what to do? I don't want to get an RFE. Thanks a lot.

Try getting the copy from them. That's normally not a problem. A copy of the complete medical, on the order hand, can often be denied. I've never seen anyone been denied a copy of the ds3025 to be sent to them when asked. Ask if they'll scan and email.

You won't ask another physician under any circumstances other than if she'd need to do more vaccinations or have a new medical in the future if you were to wait over a year after filing for an interview, for example. And that would be a form i693. The ds3025 is only vaccination supplement for the overseas medical.

Timeline

Since she got her medical in January and we're ready to file AOS within the year required, does that mean we don't have to include a photocopy at all because they match up the AOS file with the medical results submitted at the POE and I'd only need to get a new medical if they lost the results? I just read this and am double checking that my understanding is correct.

Yes, you got it. Sending the copy of the ds3025 is mostly to show them that this is the case. On rare occasions they'd send an rfe for a medical from a K1 that isn't required. The ds3025 can help show that a medical is supposed to have been linked up to the applicant's A-file. In my cover letter list of the aos package for this item I wrote something similar to

 

× Copy of DS-3025 received upon performing the overseas medical and showing US vaccination requirements COMPLETE.

 

not exactly in those words.. but you get the jist.... I'm sure you don't need to do that,  and you shouldn't be requine to but I was trying to be proactive, in case I got an IO with less K1 AOS experience.

Looks like you are in the same position as we were

You were lucky enough to have a photo. Nobody told my fiance that he should have received a copy and the doctor never kept a copy in his records to send him either :-/.

 

We sent the I-485 without the DS-3025 on 2/8 and instead wrote a letter explaining the situation. So far we have not received any RFEs (fingers crossed). If we are lucky the border agent sent in the paperwork and the vaccination form was matched to our AOS application...to be seen.

Timeline

Just want to thank you both.

 

I read your topic right before I had to get my medical results yesterday (it took more than two weeks for me to receive them). When I finally got there, the doctor was out, and he had closed the envelope.

I was happy that I received them, got out, remembered this topic, then returned and explained the situation about the vaccination card.

I was clearly not the first person to ask this, as they nodded saying "oh another one" to each other in French. 

 

I asked the medical center if they could open my closed envelope (it wasn't really sealed, just closed; didn't have a name or anything on it). They said that's fine. 

They then gave me another envelope I could close myself, and they put a stamp on the part where it should be closed, so I can seal it myself ("we trust you", they said. And they can. Also, they're my records lol).

So right now I have my medical record, and I made a decent scan, and printed it off.

 

I was also able to insert an English translation of my vaccination proof (which the doctor had input himself, as I brought copies for him, but they're in Dutch), and an English translation of my last consultation with my neurologist confirming as long as I take my meds, I can continue my life as normal. Again, the doctor put the Dutch ones in there himself, so I'm assuming inserting an English translation is fine?

 

I then scanned everything in there for my records, and printed it off as well. Everything except for the lung dvd because that one is in a sealed DVD envelope, which I don't want to open.

 

Is there anything else I need to do before I close it?

 

 

And thank you both for warning me.

Despite knowing it up front, the whole situation still stopped me from asking a copy from the doctor (when I was there, I had to give a urine sample first, and in the meantime he had other clients; I was asked to give it somewhere else, and there went my chance to ask for a copy; and when I picked up the results, he wasn't there to do this for me.)

I think the entire situation is so weird and new, that many people forget about this.

 

If I hadn't read this right before I went in, I might have just ended up stupidly walking out, happy that I finally had my results.

 

(My interview is on Tuesday.)

Look at the photo you have and look for the phrase "blanket waiver request" or something down those lines (at the bottom somewhere) if that item is checked, print the photo and include it.

If it is not, don't include it.

 

Meanwhile try to hunt down the form in Belgium and have it ready just in case you do get an RFE for it.
