How Part 8 info should appear?

Ok, so after all my searching, I have not found any posts regarding how the additional info should look.  I only wrote down the letters that have info. Letter G isn't filled out because I need to reverify with her what the zip code is. But its all about her work history, so naturally it starts on Page 5 on Part 2, and it begins with Question 16, so I take it that is the Item Number as well?

I'm sure you can write that way - on similar forms I've written the item No as an interval. So if the work history is items 16-18 (can't remember if it's really 18) I'd write it like that. I also don't think you have to write the letters, just do it any way that fits - but again, I'm also sure that you can do it that way if you want to. If it mattered more, it would be specified :)
