Hello everyone.  I'm in a huge dilemma here. I am to file AOS in July, and I'm not so sure whether to include I-693 with I-485 packet. I came to US Sept 2013, so the medical exam that I took from my own country is obviously expired as it has been more than a year. And I'm also out of status. So, I know there are other topic about this on the forums, but I would really like to hear from anyone who just filed recently, or to those who had the same problem - who filed without it and didn't have any worries, or to anyone who simply have an idea about it. Also, does that form I-693 is about to be filled out by me too, or just the surgeon? If so, do I have to bring that forms when and if I go to the civil surgeon or he/she will provide it? Thank you for any ideas and opinions that you may give me.

The i693 is required for the AOS - If you don't send it with the application you will be either rfe'd for it or asked to bring it to the interview. Regardless you will have to have it done. There are portions you fill out and portions CS fills out but you fill it out there. This is all described in great detail in the instructions. You can't miss it.

I forgot to put that my concern too is that if I filed the I-693 together with the I-485, and it's only valid for a year, which means that if the USCIS take long for my case ( over a year ), does that mean that I will have to take another medical exam? So, I know they will RFE me, but not sure what's best to do. I also read what you said about me filling out the I-693, my question is, do I have to bring the forms to the civil surgeon or they will provide them?

That's a possibility and why mny send when requested or bring to interview.

They are supposed to have them. You can always call and ask if it worries you :)
