You only need to put your current address in address history if it is different from your mailing address. 

 

Address history is page 2, part 2, and Employment history is page 4, part 2.  I would leave out item number for these, as they aren't expanding on an item already there, they are addtional information. Just put a note in the lines: "Additional address history" or "additional employment history"

Removal of Conditions

08/14/2019: I-751 mailed
08/20/2019: I-751 delivered to Phoenix
08/26/2019: Check cashed 
08/29/2019: 18 month extension letter received 
01/08/2019: Biometrics completed
 

Naturalization

07/29/2020: N-400 submitted online

12/21/2020: Interview date

Sorry I still did not get Address History Part.

What will be Physical Address 1 if one has not stayed in current address for 5 years. Shall it be blank and start typing from Physical Address2 ?

I would put the address you have in Physical Address 2 in Physical Address 1. Since you can put a start and end date, and you indicated in 11. that your physical and mailing address are the same, they will know that the items listed in the Physical Address section are previous addresses.

Yes, put the physical address or write same address just don't leave anything blank it will be rejected
