How do you if you will have a name change or not during the n-400 interview? My last name consists of two parts e.g. Mac Cormick but I have also used MacCormick due to field limitations on the ID.  He said he will use the one without the space. I asked if this will require a judicial ceremony and he said no. Do you sign an extra form when you request the name change? I only signed one paper with the information to put on the certificate and the n-400. 

 

Any input is greatly appreciated.
