How To Fill Certain Sections of i-751 and other RoC questions

Hi All,

 

I have a question related to what needs to filled in a particular section of the i-751. In Part 7., there is a section called "Acknowledgement of Appointment at USCIS Application Support Center". Under this section are we supposed to type or write our full name??

 

Similarly, under Part 8. do we enter my Spouse's full name?

 

Under Part 4., Section 5. A-Number (if any), do we need to enter the Alien number even though my wife is now a naturalized citizen? 

 

How many months worth of bank statements and utility bills do we need to produce as proof of evidence?
