Is this everything that is required for filing AOS/EAD/AP? (Cover letter within)

Hey,

 

I thought I would attach my cover letter for my AOS/EAD/AP application here, for people to be able to use it themselves and also to get some feedback if I am missing anything or have anything that is not needed.

Dear Sir/Madam:
Please kindly find enclosed concurrent filing of forms: I-485, I-765, and I-131.
Contents include:

1. Check of $1225 for form I-485 made payable to Department of Homeland Security (including Biometrics fee of $85) 


2. I-485 Application To Adjust Status 
• Check of $1225 made payable to Department of Homeland Security
• Form G-1145 (e-Notification of Application)
• Form I-485 signed by applicant
DS-3025 Vaccination Record completed fully, (I-693 Medical Exam less than 1 year old turned in at POE)

• Copy of passport's biographical page 
• Copy of passport's American visa page and entry stamp
• Copy of I-94 form 
• Copy of Marriage Certificate 
• Copy of Birth Certificate
• G325A signed by applicant
• Two (2) Passport-style photos from Applicant and Two from petitioner (placed in a small plastic bag, labeled with full name & birth date)

 

3. I-864 - Affidavit of Support from the petitioner US Citizen. 
• Copy of Petitioner’s employments letters showing annual income from June 15th 2017 (not employed yet – was a medical student and starting pediatrics residency on June 15th)


4. I-864 - Affidavit of Support from Co-sponsor US Citizen.
• Copy of Co-sponsor’s W-2 Wage and tax statement for the year 2016
• Copy of Co-sponsor’s letter of employment stating annual income

• Copy of Co-sponsor’s birth certificate


5. I-765 - Application For Employment Document 
• Copy of I-94 form of applicant
• Two (2) Passport-style photos from Applicant
• Photo copy of biographical page and U.S. visa page



6. I-131 - Application for Travel Document (Advanced Parole)
• Copy of I-94 form of applicant
• Copy of passport biographical page and U.S visa page
• Letter explaining reasons to ask for an Advanced Parole
• Two (2) Passport-style photos from Applicant


Copies of documents submitted are exact photocopies of unaltered documents and I agree to submit original documents at a later date if required.

I shall add the tax transcript for main sponsor and birth certificate of USC and sponsor.

 

Also, is adding any "proof of relationship" (pictures etc like in I-129 form) required?

You don't need to send passport photos of your USC spouse.

If it isn't difficult, it isn't worth it.

At the AOS stage it is not required to send photos like you did in the I129F.  You should start a file and collect documents and photos of an ongoing relationship and proof of commingling of a marriage in case you get an interview, or for the ROC stage.

Timeline

The only "relationship evidence" that is required with the initial AOS filing is the marriage certificate. If you are called for an interview, you will be asked to bring pictures and proof of financial co-mingling.

If it isn't difficult, it isn't worth it.

One other item...agree with rest of suggestions.

 

"I-693 Medical Exam less than 1 year old turned in at POE".

 

I-693 is typically not turned n at POE. Normally vaccine record DS-3025 is the one that most petitioners bring with them to US.

At the very top, no need for your spouse's name to be listed. But include your address and A-number.

You first say passport's American visa page, and then U.S. visa. I would recommend 'K-1 visa' for both.

I also included copy of NOA2 of I-129F approval with I-485 (and with I-131).

I read somewhere in the forum and also in the guides I think, that it is important to say I-485 Immediate Relative (Spouse) Family Based Adjustment of Status Application.

For AP, yes you do not need an explanation letter but if you do include one, you should call it Supplement for part 7.

Also yes, you do not need to include photos of the petitioner/USC spouse and you also do not need to mention in the cover letter that they are labeled and in a plastic bag.

Well it shouldn't say "i-693" cause that is the specific medical form that's done inside the US. The OP did have an overseas medical though together with their vaccinations - this is turned in at POE. 

OP you should just write "overseas medical"

Plz am confused about Alein number. I wasn't issued any I-94 form at the POE. I tried to fill out AOS form and their is a space for it

You can find your alien number on the NOA 2.

Like @DanniColes said, you can find your Alien Number on NOA2 for the I-129F, I remember it being on the right side in one of the boxes.

You should print your I-94 from the website mentioned above, and you might also have an oval stamp in your passport with a date when your 90 days are/were over, you can also include a copy of that page.
