Help renewing 10 year resident card. Need help with a question.

My 10 year card expires in july. I'm going through the I-90 form and It asks for the date on which I was granted granted lawful permanent residency. Is that the date on my greencard that reads Resident since: 04/05/2005? 

 

I also need help answering two questions in I-90 form instructions in Part 3 which I listed bellow.

"...1.  Location where you applied for an immigrant visa or adjustment of status. List the location of the U.S. Embassy, U.S. Consulate, or USCIS office where you filed your application for an immigrant visa or adjustment of status.

...2. Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status. List the location of the U.S. Embassy, U.S. Consulate, or USCIS office where you were issued an immigrant visa or granted lawful permanent resident status" .

I'm in Pennsylvania, so far my Adjustment of Status and applications were always mailed to  Vermont Service Center which is based in St Albans VT.    I have a  notice of action for my  I-751 petition to remove conditions on residence and receipts, as well as a letter that came after that in May of 2005  saying"welcome to the USA",  letting me know that a "new permanent card" was on its way. All of these came from the Vermont Service Center. So is this the USCIS office?
