My head is spinning from filling in forms, reading and re-reading and making copies.

On a K-1 visa entry is the I-693 Medical Exam form required to be filed with the AOS. I have read all you need to do is send in a copy of the vaccinations received ??

Also I have read that all we need to submit from my wifes passport is a copy of the K-1 visa or do they want the entire passport copied ??

It depends. Basically, they ask for the "vaccination supplement" form, which is only filled out by a US civil surgeon. If you want to be least likely to get an RFE, you bring the records of the vaccinations received at the initial exam (hopefully you have the "vaccination worksheet" from the foreign civil surgeon, which is different from the vaccination supplement that you get in the US) to a designated civil surgeon in the US. You will pay somewhere between $10 and $200 to have this filled out, and you may need to shop around to find someone willing to fill in the form without repeating the medical (you do NOT need to repeat the medical if it was done within the past year).

Some people simply send in the vaccination worksheet that they received at their overseas medical exam. Some of these people get an RFE asking for the supplement, some have no problem.

Some people simply include a letter stating that vaccination information was done at the initial medical in the foreign country and that the information was surrendered upon entering the US. Again, some people get an RFE, some people have no problem.

So basically you have to decide how much money you're willing to pay and how much of a risk you're willing to take.

You need to copy the visa page, the face page of the passport, and both sides of the I-94; make multiple copies of each if you are submitting the applications for EAD and AP.
