getting my son his passport (born in Ukraine with placeholder name in father position due to single mother)

I'm writing in the context of my wife is now a US Citizen born in Ukraine.  We are trying to get a passport for her son (I'm step-parent).who is 12.  She was a single mother when he was born and on the birth certificate they placed a placeholder/fake name that included my wifes maiden name as the last name and a made up name for the first and Patronymic name.  On the birth certificate this fake name has no country associated with the name as it isn't real and it also references the Ukraine family code section.  I looked up the family code and it mentions what is done when there is a single mother and it matches what my wife told me as to why there is a dummy/fake name in the father position on the birth certificate.  

What advice do you have ask to handle getting my son his US passport now that my wife is now a US citizen.  We have his original birth certificate and we are going to get a certified translation of this original birth certificate.  Thanks in advance for your help.

How did you obtain a immigrant visa for the child without parental consent?

I believe she got a "Certificate of No Father" (single mom certificate) and used that.  She told me she had no trouble in the process.  She only had problem at Boryspil airport but showed them the certificate.  She doesn't know where this document is now.  I filed away all her documents but I don't remember filing or scanning this one.
