Another issue had came up

When my father won the DV Lottery in 2005 he did apply for AOS and had the medical exam done. Do they have to do another exam now?

The problem is that we all do not remember the doctor who did it, and it would be easier to just go back to him and have everything transpose from the old records.

I know that it states that the medical exam does not need to be submitted with AOS, but I would like to avoid any RFE that may came up to save time.

Is it possible that the USCIS has the record of the exam or at least the physician name that completed it?

2005 is so long time ago. Now is 2017.

Medical exam validity does not last forever.

Starting June 1, 2014, USCIS will limit the validity period for all Forms I-693, Report of Medical Examination and Vaccination Record, to one year from the date of submission to USCIS. Applicants must also submit Form I-693 to USCIS within one year of the immigration medical examination. USCIS will also provide additional ways to submit Form I-693. As outlined in policy alert PA-2014-005 (PDF), this updated policy applies to any Form I-693 supporting a benefit application that USCIS adjudicates on or after June 1, 2014.

So yes, your father needs a new medical exam by USCIS approved civil surgeon, with all the updated vaccination shots if required.
