Will it affect the waiting time if I don't include the medical exam? I answered this in my first answer. You might get rfe'd for the medical if It's not there. Which would extend the time you'd have to wait.

They sometimes rfe, they sometimes don't and ask for it in case an interview is booked. 

There are certain immigration doctors that do this - only they are allowed  there is list on uscis website. They know what will be required for the exams. The form is i-693. You will receive it in a sealed envelope not to be opened, to send with the AoS.

Inaccurate information. In this particular case yes the I-485 gets sent to the Chicago Lockbox.

 

However, ALL AOS petitions do not go to the Chicago Lockbox first. Where an I-485 gets sent depends on the basis of filing - very important to clarify that. The following USCIS web link clarifies which I-485 gets sent to where:
