Need help,I am rescheduling my oath ceremony due to family emergency and it says on the paper that I have to sent back the original copy of Oath Ceremony letter and an explanation letter why I cant come on the given date. Now im wondering if I have to answer the back page of the paper before returning it back. Thanks in advance.

No, you shouldn't have to - they'll send you a new one when they reschedule your oath ceremony, and the answers to those questions have to be current when you actually go to take the oath.
