Yo, has anyone done this? He got his physical done in London just a few months ago so now he just needs vaccines here in America. 

 

From my understanding on the I-693, the civil surgeon will fill out only a few sections and the physical will be left blank. Since uscis has the physical exam results, we don't need to send anything in for it? He was given a disc to guard with his life because it's proof if he needs it... Is this correct? Or does this civil surgeon need to transcribe the results? 

 

Do you have to get a blood test to determine what vaccines you need or just go off your word?

The medical check-up back in London should be valid for a year. So if you file for AOS within that period, you only need to submit a copy of the DS-3025 provided that his vaccinations are all done. If your filing of AOS is beyond that one year period, you'll then need to get the DS-3025 transcribed onto a I-693. In the event that he requires additional vaccinations, it is not an issue at all for him to get them in the US. Make sure he has all the vaccines that are required.

The disk is the chest x-Ray and you don't need to guard it, nor do you need it for anything really. They give it to you because neither Knightsbridge or embassy want to store it. A radiologist written report of it is in you medical exam results and that's what cleared you of TB for the visa. The same medical report can be used for your AOS. USCIS will have it in your file.

 

A civil surgeon will fill out the immunisation part of the I-693 since you didn't get complete at Knightsbridge.  Read the form instructions carefully so you fill out your part correctly. The CS can use records you take of jabs that come from your GP, the form from Knightsbridge, a travel clinic, a US Health Dept, or any pharmacy that offers immunisations. Or you can pay him extra for any shots you know you need. He puts all your shots from any source on the I-693. He should give you a copy to keep and seal the original up (as described in the form instructions) for you to send with AOS.

 

You do not need another exam or TB test, but many civil surgeons will say you do. Call around asking prices and making sure they will only do the immunisation part before you choose one. Prices vary a lot.
